A majority of patients with COVID-19 are adults. Among 44 672 patients in China with confirmed infection, 2.1% were below the age of 201 . The most commonly reported symptoms included fever, dry cough, and shortness of breath, and most patients (80%) experienced mild illness. Approximately 14% experienced severe disease and 5% were critically ill. Early reports suggest that illness severity is associated with age (>60 years old) and co-morbid disease.

Clinical care of patients with COVID-19 focuses on early recognition, immediate isolation and implementation of appropriate infection prevention and control (IPC) measures; provision of symptomatic care for those with mild illness; and optimized supportive care for those with severe disease. WHO has published patient management guidance, including interim clinical care guidance for hospitalized patients and home care guidance for those with mild disease that may be treated at home in isolation when the health system is strained.

Oxygen therapy is the major treatment intervention for patients with severe COVID-19. All countries should work to optimize the availability of pulse oximeters and medical oxygen systems. Mortality in those with critical illness has been reported as over 50%, thus implementation of proven critical care interventions such as lung protective ventilation should be optimized. 1. Globally: The term “primary health care” (PHC) first emerged in the United Kingdom in the 1920s with the publication of a government white paper, the “Dawson Report”. The report suggested that PHC centres would become the model for providing community health care services as a strategy to address health inequalities and respond to the increasing complexity of health care delivery (1). Over the following decades, the concept of PHC developed significantly, culminating in the 1978 Alma-Ata Declaration, which defined PHC as “essential health care based on practical, scientifically sound and socially acceptable methods and technology made universally accessible to individuals and families in the community through their full participation and at a cost that the community and country can afford” (2). Now, 40 years later, the Global Conference on Primary Health Care reaffirms the global commitment to PHC, as a key strategy to achieve universal health coverage and the United Nations (UN) Sustainable Development Goals (SDGs) (3). To date, the implementation of PHC internationally has been limited by the lack of a universally accepted definition. The background paper for the Global Conference on Primary Health Care, A vision for primary health care in the 21st century, aims to resolve this issue by describing PHC as whole-of-society approach to health, based on three interrelated and synergistic components.

2. North-America Region:

For the past 40 years, the Declaration of Alma-Ata has had a strong impact on the formulation and implementation of health policies and the development of health systems in the Region of the Americas. The Declaration’s call for a new civilizing order, the reaffirmation of the right to health, equity and social justice, the responsibility of governments to achieve health for all, the need for a new way of organizing health services, make knowledge and health services accessible to people and communities, the call to participate, among many other contributions, have continued to inspire and guide many of the efforts to guarantee the right to health. This regional report briefly describes the main elements that characterize the implementation of the Primary Health Care (PHC) Strategy in the Americas. Following the guidelines and the thematic order suggested by WHO, the report focuses on the last 10 years; although, in the case of the Region of the Americas, it is necessary to refer to the predating processes. The report begins by describing the regional mandates regarding the transformation of health systems inspired by the Declaration of Alma-Ata and the effect of these mandates in PAHO’s strategic planning processes that guide technical cooperation. Next, it addresses the regional results, enabling factors, and obstacles encountered, as well as examples of adaptation at the national level, and highlights examples of countries that demonstrate significant achievements.

3. Europe:

Important drivers for health systems development are the need for greater responsiveness to the population’s health needs and greater cost effectiveness. Healthcare systems should produce better health as set forth, for example, in the United Nation’s Millennium Development Goals, the World Health Assembly 2009, and the World Health Report 2008. Strong primary care contributes to a better functioning of healthcare systems overall. This was, for example, acknowledged by the recent Affordable Care Act and the introduction of medical care homes in the US. Primary care is the first level of professional care service, where people present their health problems, and where the majority of the population’s curative and preventive health needs can be satisfied. Current challenges of healthcare systems include new risk factors, ageing populations, health inequities, increasingly complex needs, rising healthcare expenditures, technological change, and a lack of human resources. Supported by a growing body of evidence since Starfield’s study in 1994, decision makers increasingly use the strengthening of primary care in their strategy to cope with these challenges.

This study has built on previous studies that have evaluated primary care in individual countries, or made international comparisons of a limited set of functions. A major challenge in international comparative studies in primary care is to develop a suitable definition of primary care that can capture the variety in prevailing organisation and service-delivery models.

This study seeks to overcome prevalent limitations in primary care research, by comparing the strength of primary care in 31 European countries, using a standardised measurement instrument.

4. Asia pacific:

A central feature of the universal health coverage (UHC) agenda is strengthening primary health care (PHC). High-functioning PHC systems are considered essential to achieving all 17 goals (Sustainable Development Goals, SDGs) of the 2030 Sustainable Development Agenda; not just the health-specific SDG3. Yet, decades after the Alma-Ata Declaration on PHC in 1978, many communities in low- and middle-income countries (LMIC) still experience limited access to safe, effective and affordable PHC.

The growing burden of non-communicable diseases (NCD), combined with an intensifying risk of infectious disease outbreaks, presents a ‘double threat’ to fragile health systems of LMICs of the Asia-Pacific region. Here, NCDs account for 80% of all deaths and 50% of premature deaths, a result of longevity, increasing urbanisation, economic growth and lifestyle factors. At the same time, outbreaks of emerging diseases such as severe acute respiratory syndrome (2003),Influenza A (2009) and Zika (2016) are likely to continue. A robust PHC system underpins a nation’s ability to promote population health, and remain resilient in times of crisis. Yet PHC systems in many Asia-Pacific LMICs are the ‘poor cousins’ of the hospital system. Designed primarily to provide episodic care for acute disease management, they often struggle to manage long-term conditions, deliver preventive health activities or to integrate care across diseases.
